Fees
The board has established the following fees for testing and licensure.
- Application for Licensure — $500
- Renewal Fees (annual) — $350
- Replacement License — $10 and a written request
- Corporation Registration — $50
- Corporation Annual Renewal — $10
All fees are non refundable.
