To request verification of a South Dakota licensed or registered Pharmacist, Intern, Pharmacy Technician, Pharmacy or Wholesale Drug Distributor, click on the appropriate link below whether looking for an Individual or a Business.
If you have a verification form for our office to complete or need a separate verification form you may email, fax (605-362-2738) or mail it to our office. There is no charge for verifications.
