Prepare for Your Appointment

  • Schedule your appointment ahead of time.
  • Bring the following items with you:
    • A current government-issued photo ID.
    • A taxpayer identification number, such as a Social Security number.
    • Any other documentation you need for your appointment.
  • Use the CDC COVID-19 Facility Access Tool to protect yourself and our employees. If you are feeling ill, please reschedule your appointment.
  • If your area has a high transmission rate or you aren’t fully vaccinated, you must wear a mask.
  • Stay 6 feet away from others.
