generating new sub domain in a WordPress multisite installation

I have a big issue with a wordpress mu site. and before you say, start again with a fresh installation, I cannot because the site is a number of years old.

SiteX is a multisite installation. When I create a new sub domain site say: i.e. test45.sitex.com its says Site added. Visit Dashboard or Edit Site. But the link to the dashboard points to route domain i.e. Sitex.com/wp-admin and the visit dashboard points to the sitex.com dashboard.
The link under Site Address (URL) is :site.test45.com/ which when typed into the browser says Error establishing a database connection.

WordPress expert wpfixit gave up after a couple of hours of trying to solve the problem.

Does the issue still happen with all plugins disabled and a default theme (Twenty Seventeen) activated?
