This question type allows you to capture a respondent’s name.
Options
Here’s what the name question type looks like in the survey editor:
Include a note – Checking this will allow you to enter a note that will appear under your question.
Question is mandatory – Checking this will make the question mandatory, meaning that it must be answered before the participant can complete the survey.
Display Fields – Changing the name format on the dropdown determines whether title and suffix are captured. You can choose from full name, title, first and last, or first, last
Note that you can add media to the the name question type.
