This is by far the best plugin for MailChimp. I have Gravity forms extension, WooCommerce MailChimp extension and even that official one in the Repo but none offer customization as this.
With the form builder its lot easier to style it with any CSS framework like Zurb's Foundation or Bootstrap by Twitter.
