Display form from PHP

I need to add 1 of several forms on a page depending on the $_REQUEST. I have tried:

do_shortcode(‘[caldera_form id=”F5ca37e19aafb8″]’);
Caldera_Forms::render_form( ‘CF57cd8747beba8’ );
echo Caldera_Forms::render_form( ‘CF57cd8747beba8’ );

I also need to set default values, but I assume I can do that with the $_REQUEST.

An example would be appreciated.
